Home Care – Person Centred support…

We provide person centred support to the elderly, the disabled and vulnerable adults who require day to day help to maintain their independence and well-being at home and within their local community.

Support at home can include:

  • Assistance with help to get up, wash, shower or bath, get dressed and have breakfast.
  • Support with general shopping, and getting out and about in the community.
  • A sitting service to allow the carer to take a break.
  • Preparation of food and drink.
  • Medication assistance – Ensuring a timely prompt for medication when due.
  • Undertaking domestic tasks such as cleaning, laundry and ironing. Clean clothes, fresh bedding and healthy meal before bedtime.
  • Assistance at the end of each day, some help with getting ready for bed.
  • Determining a Care Plan is key to delivering the right level of support for your own needs.

Churchill Health Care senior staff will complete a full risk assessment within your own home, and through consultation with yourselves and family to identify your personal requirements. This information is recorded in your Care Plan.

Your Care Plan may be made up of any combination of our support services, this could include;

  •  Personal Care
  • Help with getting up
  • Washing and Dressing
  •  Toileting
  •  Shaving
  •  Assisted bathing
  •  Medication Supervision
  •  Collecting Prescriptions
  •  Shopping
  •  Waking or Sleeping night service
  •  Befriending – companionship
  •  24/7 living in care
